Paralytic Ileus

Refers to the inhibition of propulsive intestinal motor activity in the absence of a mechanical obstruction

Aetiology

  • Recent GI surgery (post-operative ileus) - most common cause
  • Other causes include inflammation, medications, infection and diabetic ketoacidosis

Clinical presentation

Symptoms

  • Failure to pass flatus/faeces
  • Bloating
  • Nausea and vomiting

Signs

  • Abdominal distension
  • Absent bowel sounds

Investigations

  • AXR
  • Bloods and CT (abdo and pelvis) to rule out more serious pathologies

Management

  • ‘Drip and suck’ while awaiting restoration of peristalsis - NG tube feeding and IV fluids
