Calcaneus Fracture

Aetiology

  • Usually follows axial compression e.g. falling from height onto the heel
  • Often intra-articular fracture

Clinical presentation

  • Pain, inability to bear weight
  • Significant swelling
  • Look for other injuries especially spinal

Investigations

  • X-ray - calcaneal compression causes loss of the central peak seen in a normal calcaneaus (measured using Bohler's angle), and bone density will increase
  • CT - calcaneal fractures are often comminuted, anatomy can be clarified by CT

Management

  • Cast immobilisation with nonweightbearing for 6-12 weeks
  • Surgery controversial - high risk of infection/wound breakdown and benefit not proven

Complications

  • Risk of compartment syndrome
