Usually follows axial compression e.g. falling from height onto the heel
Often intra-articular fracture
Clinical presentation
Pain, inability to bear weight
Significant swelling
Look for other injuries especially spinal
Investigations
X-ray - calcaneal compression causes loss of the central peak seen in a normal calcaneaus (measured using Bohler's angle), and bone density will increase
CT - calcaneal fractures are often comminuted, anatomy can be clarified by CT
Management
Cast immobilisation with nonweightbearing for 6-12 weeks
Surgery controversial - high risk of infection/wound breakdown and benefit not proven
