Tibial Shaft Fracture

Aetiology

  • Low energy injury - result of indirect torsional injury
  • High energy injury - result of direct force

Pathophysiology

  • Multiple fracture configurations - spiral, transverse, oblique, comminuted
  • Open fractures more common
  • Higher risk for compartment syndrome

Clinical presentation

  • Pain
  • Inability to bear weight
  • Deformity

Investigations

  • X-ray - AP and lateral

Management

  • Conservative - above knee cast
    • Patient may need closed reduction in theatre before cast is fitted
  • Operative - IM nailing, ORIF
